Twofold AI, sold as Twofold Health, is an AI medical scribe. It listens to a visit, in the room or over telehealth. Then it writes the clinical note in the format your specialty uses. It is self-serve, with a free week and a signed BAA on every account. That is why therapist and small-practice forums name it as the cheap alternative to enterprise scribes. The site says more than 20,000 clinicians use it.

What does Twofold do?

You capture a visit one of four ways. Record it live, dictate a summary afterwards, type rough notes, or upload an audio file. Twofold transcribes it, separates speakers, and produces a structured note. SOAP, DAP, progress notes, H&P, discharge summaries, referral letters and treatment plans are all templates. The app store listing counts more than 50 formats. Copilot Mode turns one session into several documents at once. A SOAP note, a referral letter and a follow-up email to the patient can come from one visit.

The specialty coverage is wider than most scribes. Beyond primary care and mental health, the templates cover paediatrics, physical and occupational therapy and speech-language pathology. Dietitians, lactation consultants, dentists, aged care and veterinary work have templates too. Notes carry CPT and ICD-10 code suggestions. Patient tracking keeps trends like PHQ-9 scores, weights and blood pressure across visits.

It learns your style. Edits you make feed back into tone, section order and saved phrases, so later notes read more like you. Multi-speaker sessions, such as couples or a parent with a child, are attributed correctly, which several reviewers single out.

Twofold runs in any browser and has iOS and Android apps with cross-device sync. It works with any EHR in the sense that you copy or push the finished note across.
